RaphaCare Terms and Conditions

RaphaCare is a medical membership program offered by Rapha Medical Center that provides enhanced access to certain primary care services for a fixed monthly fee over a one-year membership term.

RaphaCare is designed to support relationship-based, preventive-focused medical care and is not health insurance.

• RaphaCare is a one-year (12-month) membership commitment
• Membership fees are billed either monthly or annually, depending on the option you choose.
• Membership benefits apply only during the active membership period
• Plan features, pricing, and benefits may be updated annually and will apply at renewal

By enrolling, the member agrees to the full one-year term.

Monthly pricing is as follows:

• Individual: $125 per month
• Family of Two: $210 per month
• Family of Three: $300 per month
• Each Additional Family Member: +$90 per month

Family memberships apply only to individuals residing in the same household.

All fees are subject to applicable taxes where required by law.

• New members enrolling during the months of February and March are eligible for a $100 enrollment discount by using the code SPRING100 at checkout.
• The discount may be applied to the first billing cycle or total membership value, at the discretion of Rapha Medical Center
• This promotion is limited-time and may be modified or discontinued without notice

Each enrolled member is eligible for the following services during the membership year, subject to medical appropriateness:

Unlimited Medical Consultations

Includes medically appropriate:

• Phone consultations
• Provider follow-ups and care coordination

Unlimited access does not include emergency services, non-medical administrative requests, or services outside provider scope.

Four (4) In-Office or Tele-Video Visits Per Year

• Sick visits
• Chronic condition management visits
• Preventive care visits

Unused visits do not roll over to the next membership year.

One (1) Complete Lab Panel or Ultrasound Procedure Per Year

• Provider-directed laboratory testing
• Lab selection is determined by medical necessity
• Labs outside the included panel may incur additional charges

RaphaCare does not include:

• Emergency room care
• Hospitalization or inpatient services
• Ambulance services
• Specialty care outside Rapha Medical Center
• Imaging, procedures, or labs not specifically included

Members are strongly encouraged to maintain health insurance for services outside the scope of RaphaCare.

• All medical services are provided based on provider judgment and medical appropriateness
• Membership does not guarantee availability of any specific test, medication, or treatment
• Providers retain full professional discretion in all care decisions

• RaphaCare is not insurance
• RaphaCare does not replace health insurance
• Rapha Medical Center does not bill insurance for RaphaCare membership fees
• Members are responsible for any services received outside the membership scope

• Monthly payments are required to keep the membership active
• Missed or failed payments may result in:
  • Suspension of benefits
  • Termination of membership after notice
• Reinstatement may require payment of past-due balances

• RaphaCare is a one-year commitment
• Early termination policies, including any applicable fees or remaining balance obligations, will be outlined at enrollment
• Rapha Medical Center reserves the right to terminate membership for:
  • Non-payment
  • Abusive behavior toward staff
  • Misuse of services
  • Violation of these Terms & Conditions

• Family members may be added mid-term, subject to updated pricing
• Benefits are not transferable to non-enrolled individuals
• Membership benefits apply only to the named enrolled members

• All medical information is protected under HIPAA and applicable privacy laws
• Membership status does not alter patient rights to confidentiality or access to records

Rapha Medical Center provides medical services consistent with professional standards of care. Membership participation does not create guarantees regarding outcomes, diagnoses, or treatment results.

Rapha Medical Center reserves the right to update these Terms & Conditions as necessary. Any material changes will be communicated to members prior to renewal.

By enrolling in RaphaCare, the member acknowledges that they have read, understood, and agree to these Terms & Conditions.
